SEATTLE — The Washington Department of Labor &Industries has fined a Seattle marine terminal operator more than $400,000 for more than 50 serious safety and health violations at its facility on Harbor Island.
The state began investigating Seattle Bulk Shipping after a worker was hospitalized with injuries when he fell into an underground grain storage pit in December 2014.
The $424,850 fine is among the largest Labor &Industries penalties of a business in recent years.
The violations cited include not ensuring personal protective equipment was available during an emergency response to a possible ethanol fuel release and other problems with emergency response plans.
