PORT ORCHARD — State regulators have fined a Port Orchard assisted living facility for multiple violations.
The Kitsap Sun reports that the Washington Department of Social and Health Services also placed conditions on the facility’s license for the third time in three years.
Park Vista Retirement and Assisted Living Community was notified of the discipline at the end of August.
The violations included failing to properly monitor residents and not ensuring proper oversight for delivering medications. They also were cited for failing to investigate safety incidents involving four residents.
The agency fined Park Vista $1,400.
Park Vista also was disciplined in 2013 and 2014. The 70-bed facility is the only one of Kitsap County’s 23 assisted living facilities to have conditions on its license this year.
