Fodor home license will be revoked

The owner of Fodor Homes, Susan Fodor, was issued a letter from the state indicating her license will be revoked.

The letter was hand-delivered to Fodor on Tuesday, Aug. 1, by a foster home licenser. This was the day her probationary license expired, said Mike Tornquist, state Department of Social and Heath Services (DSHS) administrator of the Division of Licensed Resources.

“This is a notification of the intent to revoke her license,” Tornquist said.

Fodor owns Fodor Homes, LLC, a cluster of group homes in Lake Forest Park, in the vicinity of 40th Avenue NE. The homes catered to about 29 autistic children.

Fodor is entitled to a hearing to dispute the licensing action, Tornquist said. If she wishes to contest the decision, she may do so by requesting an administrative hearing in writing.

The request must be sent and received at the office within 28 days of the day Fodor received the letter, Tornquist said. Failure to request a hearing in 28 days will result in a final revocation of her license.

In early June, Fodor closed all but one of nine homes in Lake Forest Park as part of an agreement with DSHS staff, following a deadline to comply with state ordered changes at the homes after an investigation. Fodor was unable to bring staffing levels, meals, safety preparation and documentation of prescription medication up to standard.

On July 19, three children were relocated from the remaining home in Lake Forest Park along with three children from a home in Snohomish County.
